About SWMC’s Patient and Family Advisory Council (PFAC)

We have invited patients and caregivers across our community to share their feedback about staying connected with their healthcare providers. The Patient Advisory Council serves as a “consumer advisory group,” providing a way for patients and families to provide input and share their experiences.

Our goals include:

  • Expanding the voices of patients and families
  • Identifying opportunities for improving the patient and family experience
  • Improve quality and safety of care
  • Enhance patient and provider satisfaction

What does a PFAC member do?

Patient and Family Advisory Council members are invited to participate in focus groups to share their feedback and join in-depth discussions about their healthcare experience.

Our Commitment to Diversity, Equity, and Inclusion

SWMC’s commitment to Diversity, Equity and Inclusion helps guide our decisions to address the needs of our patients. We welcome members of all races, ethnicities, genders, religions, ages and backgrounds to help us serve the diverse needs of our patients.

How Can I Join?

If you have constructive ideas to help improve care for SWMC patients and their families and/or you or someone you care for sees a SWMC provider, we invite you to join as a SWMC Patient and Family Advisory Council member.

Your voice matters to us and will help us improve the healthcare experience across our medical group.

If you are interested in additional details about the council, or if you would like to join, please email jvazquez@swmedcenter.com or fill out our online application.
